What the Hook? – A deeper understanding of Hooks in WordPress

WordPress is built with the assumption that plugins and themes are going to extend its core functionality. This is done through the system of hooks that come in the form of filters and actions.

When to use action and when filter? What is the difference, if there is any? Why hooks sometimes don’t work and how to use them properly? Should you create your own hook and why would anyone do such a thing?
